Alzheimer’s research is rapidly shifting toward blood-based biomarkers that detect neurodegeneration earlier and more cost-effectively than cerebrospinal fluid (CSF) or PET imaging.

Unlike conventional plasma tau assays, BD-pTau tests selectively measure CNS-specific tau isoforms, improving specificity for true neuronal injury while minimizing peripheral interference.

Tau protein stabilizes neuronal microtubules. In Alzheimer’s disease (AD) and related tauopathies, tau becomes abnormally phosphorylated and released into extracellular fluids such as CSF and blood.
Brain-derived (BD) tau assays selectively measure CNS-specific isoforms, improving specificity for neurodegeneration relative to total or peripheral tau.

BD-pTau217 – Targets phosphorylated tau at threonine-217 enriched from CNS sources, reducing peripheral interference and potentially enhancing AD-specific signal. While early research data are promising, performance characteristics remain assay-specific and require continued cohort-level validation.
BD-pTau181 – Measures CNS-derived tau phosphorylated at threonine-181, complementing BD-pTau217 by capturing overlapping yet distinct tau pathology domains.
For reference, standard plasma pTau217 (non-BD) has been shown across multiple studies to correlate strongly with amyloid-β and tau PET imaging and to predict cognitive decline in AD research cohorts.
When run in tandem, BD-pTau217 and BD-pTau181 provide a comprehensive research framework to differentiate AD from other dementias, track progression, and evaluate therapeutic interventions, while minimizing the peripheral confounds inherent to non-BD tau assays.
